How to Enroll
Open Enrollment Period: July 14th - 24, 2026
To ensure your benefit elections are accurate and processed correctly, enrollment will be completed exclusively by phone through our dedicated Benefits Call Center. A trained benefits representative will guide you step-by-step, enter your elections for you, and answer any questions. For more information about your benefits, visit roisphoenixbenefits.com.
Enrollment steps:
1) Review the provided benefit material.
2) When ready to make benefits elections, call the Benefits Call Center to enroll.
3) If possible, be in front of a computer or smart device when calling.
4) A benefits representative will enroll you over the phone or through a video call.

Benefit Eligibility
Employee Eligibility
To participate in our Employee Benefits Program, you must meet the following criteria:

Be a full-time Employee as defined in our Employee handbook and
-
Work an average of 30 hours per week.

Enroll within the first 80 days from your date-of-hire or during annual Open Enrollment

Dependent Eligibility

Legal spouse (except for medical coverage).
-
Dependent children up to the age of 26, includes stepchildren, legally adopted children, children placed for adoption, foster children, and children for whom legal guardianship has been awarded to you or your spouse.

Dependent children, regardless of age, provided they are incapable of self-support due to a disability and are fully dependent on you for support as indicated on your tax return.
If you cover your dependent(s), benefit coverage ends at the same time the Employee’s benefit ends.

A child is no longer eligible for benefits at the age of 26. For medical, the coverage ends on their birthdate, For Dental and Vision coverage, the child’s coverage will terminate at the end of the month.
Change in Family Status
Qualifying Events allow you to make changes before Open Enrollment. When one of the following occurs, you have 30 days to contact Human Resources with the appropriate documentation to update your coverage. Your requested change in coverage must be consistent with your qualifying event.
Examples of qualifying events include:

Change in your legal marital status.

Change in the number of your dependents (Example: birth, adoption, or if a child is no longer an eligible dependent).

Death of spouse or child.
-
Change in your spouse’s employment status.
-
Change in status from full-time to part-time or part-time to full-time.
-
Entitlement to Medicare or Medicaid.
-
Qualified Medical Child Support Order (QMCSO) or legal change of custody
